Managing Gestational Diabetes During Pregnancy

Gestational diabetes mellitus (GDM) is one of the most common pregnancy complications, affecting up to 10–15% of pregnancies in Bangladesh. The good news is that with proper monitoring and care from your best female gynae doctor in Banasree, it can be managed effectively to protect both you and your baby.

What is Gestational Diabetes?

Gestational diabetes occurs when hormones produced by the placenta interfere with insulin action, leading to elevated blood sugar levels. It typically develops between weeks 24–28 of pregnancy and usually resolves after delivery. However, it increases the risk of type 2 diabetes later in life.

Risk Factors

  • Family history of diabetes
  • BMI over 25 (overweight or obese)
  • Previous gestational diabetes
  • Age over 35
  • Previous baby weighing over 4 kg
  • Polycystic ovarian syndrome (PCOS)

How is Gestational Diabetes Diagnosed?

The Oral Glucose Tolerance Test (OGTT) is the standard test, typically done between 24–28 weeks. You fast overnight, drink a glucose solution, and blood is drawn at 1 and 2 hours to measure your body’s response.

Managing Blood Sugar Safely

Diet Modifications

  • Choose low glycaemic index (GI) carbohydrates: brown rice, oats, whole wheat roti
  • Eat small, frequent meals — avoid large carbohydrate-heavy portions
  • Include protein with every meal to slow glucose absorption
  • Limit sweets, sugary drinks, white rice, and refined flour
  • Include fibre-rich vegetables: bitter gourd, leafy greens, cucumber

Exercise

30 minutes of moderate walking daily is safe and effective for most pregnant women. It helps your cells use insulin more efficiently. Always consult your doctor before starting or changing your exercise routine.

Blood Sugar Monitoring

Monitor your fasting and post-meal glucose levels at home with a glucometer. Target fasting glucose under 5.3 mmol/L and 2-hour post-meal under 6.7 mmol/L.

Medication (if needed)

If diet and exercise are insufficient, your gynecologist may prescribe insulin injections or metformin. These are safe during pregnancy when prescribed and monitored by a specialist.

Risks if Untreated

  • Large baby (macrosomia) — increasing C-section risk
  • Low blood sugar in newborn after birth (neonatal hypoglycemia)
  • Preterm delivery
  • Increased risk of preeclampsia
